Biography

Hashim Zaibak, PharmD began his career as a pharmacist in 1999, following his graduation from the University of Illinois at Chicago. With more than two decades of experience, he has seen the industry from multiple vantage points. Hashim is the founder of Hayat Pharmacy and runs 20 pharmacies in Wisconsin. In recognition of Hashim鈥檚 excellent service to the community, he was named the 2014 Pharmacist of the Year by Pharmacy Development Services. Also, he received the Bowl of Hygeia Community Service award in 2021. Additionally, Hayat Pharmacy was named Health Mart鈥檚 2014 and 2021 Pharmacy of the Year.

Research Interests

Managed a ten-month study in conjunction with ATEB, which followed Hayat Pharmacy patients that were a part of the Simplify My Meds (SMM) program versus those not enrolled in the SMM program
