Financial Aid Information for New/Prospective Students
Financial Aid Deadline
There is no formal deadline; however, students WILL NOT receive their Financial Aid Offer until all application materials have been received and their file has been reviewed.
Financial Aid Process/Offer Notification Timeline
- Submit the FAFSA
- Submit additional Financial Aid forms. For more information utilize the instructions on how to access financial aid forms on Ϲconnect (PDF).
- Completed files go through the file review process (starting early Spring)
- Financial Aid: Federal Aid Offer notification emails start going out to students (Early Summer)
- Mid-May: Financial Aid will transition to utilizing Ϲ email addresses as the main source of correspondence to incoming students
- Complete additional disbursement requirements: MPN/Entrance Counseling/Orientation Course/Letter of Appreciation (if required) (Early Summer)
- Appointment Notification emails are sent. Students will be able to schedule a required 1:1 Financial Aid Appointment if all the steps in 1-6 are completed (Summer).
- Need-based Institutional Aid awarding will begin after Orientation week and run throughout July. If you receive need-based institutional aid you will receive a subsequent Offer notification and if applicable a separate email to complete a Thank You letter.
What's New
We are sure many of you have heard about the significant changes to federal student aid. Things look very different this year for graduate students. To help you navigate the process, we developed a financing plan template to help you understand the full cost of your degree and make sure you have plan to reach graduation. Each student is required to complete the financing plan for their program.
As you develop your plan, keep in mind most students use loans to pay for graduate school unless they have other resources. There are no grants available at the graduate level. Since federal loans will not cover the full cost of tuition, you’ll want to explore your options through family, private lenders or other resources you may have in your local community. We want you to fully understand the commitment and costs BEFORE you start. You will have access to the financing plan after you have been extended an offer. However, we recommend you start planning now as it may take some organization and outreach for your plan to fully become clear.
We understand plans may change, but this will at least ensure you have all of the information and have thought through financing your degree before you make a commitment to what is likely the largest investment you’ve made at this point in your lives.
How to Apply
Financing Plan
Required for all Medical, Pharmacy, MSA, and Genetic Counseling applicants.
Deadline: May 1 (for offer extended/accepted applicants)
You will need to complete Multi-Factor Authentication (MFA)/DUO Enrollment prior to accessing this form. You will be able to access this form through . If you are offer extended/accepted after May 1 you will need to submit this form within two weeks of offer extended/accepted status.
Federal Aid Application
The Financial Aid Office receives your results electronically. The FAFSA typically opens in October; visit for more information or availability.

Financial Aid Application
The Financial Aid Application (FAA) will be available after you have completed DUO enrollment and your FAFSA has been received by Ϲ. You will be able to access this form through .
Need-Based Institutional Aid Requirement
Required for the MD Program only.
The Parental Assistance Worksheet (PAW) will be available after you have completed DUO enrollment and your FAFSA has been received by Ϲ. You will be able to access this form through . This form is student-initiated. Once you have submitted the student section of the form, parents will receive an email to complete their section. Parents will need to create a Dynamic Forms account utilizing the same email the student provided as the parental contact in the student section.

Private Loan Application
Obtain directly from lender of choice. Do not complete prior to April 1 or the application will expire before certification and a new application will need to be completed.

Wisconsin Residency Determination (MD program only)
Residency Determination Form (PDF): HEAB form processing can take several months. Do not delay. If you have additional questions, more information is available on the Ϲ Admissions webpage under How To Apply, Required Forms.
Verification Form
If you were selected for identity verification by the U.S. Department of Education, our process at Ϲ is a 2-step process:
- Complete the Verification (V4/5) Worksheet. You will need to upload images of the front and back of your government issued ID (i.e. drivers license) before submission.
- You are required to bring your government issued ID to the Financial Aid Office in person. The Financial Aid Office will send you a link to schedule a time to provide this documentation in person. If you do not complete Step 2, your funds will not disburse until the requirement is satisfied.
Authorization for Non Directory Information
If you want any financial aid information to be disclosed to any other person (i.e. parent, spouse, landlord, etc.), you will be required to fill out a Authorization for Non Directory Information form. Please contact the Registrar’s Office for this form.
Citizenship or Eligible Non-Citizenship Documentation
If you are required to provide verification of your citizenship status, our process at Ϲ is a 2-step process:
- To be eligible for file review and receive a financial aid offer, complete the Citizenship Verification form. You will need to upload an image of your citizenship documentation before submission. Examples of citizenship documentation can include the following: passport, certificate of naturalization.
- To be eligible to receive a disbursement, you are required to bring that same citizenship document you uploaded to the Financial Aid Office in person. If you do not complete Step 2, your funds will not disburse until the requirement is satisfied..
Budgeting Resources
The financial aid budget represents the maximum allowances for each category. Setting a realistic budget will be critical to your financial well-being. Be sure to complete a budget prior to committing to housing and stay within your budget. Previous Aid Year information may be posted until budget for new (upcoming) aid year is determined/released.
